Membership Cancellation Request
To cancel your membership, fill out the form on this page. Please refer to your membership agreement or read our cancellation policy below.
For those who purchased memberships prior to 9/1/23, refer back to the membership terms linked in your profile when filling out the form below.
CANCELLATION POLICY
Cancellation requests must be submitted at least 7 days before your next billing date. For example, if your billing date is the 15th, we must receive your cancellation request by the 8th of that month. Once a payment is processed, partial refunds are not available for that month.
Cancellations may not happen while on hold, and number of term payments must be made before the membership can be canceled without penalty.
Early Termination (for 3, 6, or 12-month commitments):
If you cancel before your Initial Term ends, an early termination fee of $100 will apply, unless you need to cancel for one of the following reasons:
Serious Illness or Permanent Disability: If you are unable to use the facilities due to illness or disability, we require a doctor's note or certification stating this.
Relocation: If you move more than 25 miles from any of our locations, you may cancel. If you were already living more than 25 miles away when signing your contract, you may cancel only if you move an additional 5 miles or more. You will need to provide proof of your former and new address (e.g., driver’s license, lease agreement, or utility bill).
Death: In the event of a member’s death, the member’s estate may cancel the contract with written notice and certified proof of death.
If any of the above reasons apply, please indicate this in the final question on the form. No cancellation fee will apply in these cases.
